E-mail a copy of your entry forms in Word or similar format) to the Webmaster for inclusion on the website

A reminder that entry forms will not be posted out to players unless specifically requested.

Print pages 1 & 2, then print page 3 on the back of page 1.
Page 2 can be repeated on the back of itself to give two entry forms.
